The moderation should be improved
One of my creations was moderated because of offensive/abusive content and I didn't get any message why exactly. Additionally Sony was threatening to ban my PSN account if that ever happens again. This was really frustrating.
It took me several hours to create my dream and now it's all lost. I was really surprised at first and thought it would be a mistake. It was just a simple platformer with no adult language, no blood, no sexual content or anything. But then it came to my mind that the character I imported from another dreamer must be the reason. It was an Ughandan Knuckles.
To be honest for me it was just a funny meme which I liked and although I'm aware that many people consider it as "cancer" I wasn't aware that by some people it's considered racist. I googled it and read some discussions on the internet and I get the point. For me personally I still would consider it harmless and not really racist. It's an annoying meme that makes fun of the Sonic character knuckles and some nice ughandan trash action movie. Neither vr chat nor youtube or any other platform I know ever banned Ughandan Knuckles. But I accept the code of conduct of dreams even if I think it's an overreaction.
I'd like to remove/replace the character that is being considered offensive but that is not even possible anymore since the whole dream is locked. I read that also many other players are very frustrated about the hard intransparent measures.
So my suggestions to improve this in the future would be:
When a content violates the guidelines - flag it as moderated and make it unplayable for other dreamers online. But please give an explanation why exactly it violates the guidelines. And before bringing this to Sony and threating players with permanent bans from the PSN please notify the dreamer first with a dreams notification and give the players a chance to fix it. That means the Dream should still be editable and also playable offline on the players PS4. If the player removes the offensive/abusive content then he should be able to upload the modified level again. Maybe there could be a deadline like at least 4 weeks before the dream becomes uneditable and a Sony report follows. Or it should only happen if a dream gets moderated a second time once it's uploaded again.
I really enjoy playing dreams and I don't intend to violate the guidelines. I honestly didn't know that my dream would be considered offensive. Now my motivation to create anything new is down because I don't want to create anything that might get deleted anyway because of these extreme measures. What I'm asking for is to give players a chance to fix their contents before taking any extreme measure. Please consider my suggestion.

Yeah, the Sony moderation sucks, I remember loads of random stuff on GtSport getting the dreaded 'Yellow Triangle' with zero explanation offered ever!
And it was really stupid stuff too, like helmets that were basically just one colour! 😁
I swear no actual human beings are in charge of moderation in the first instance, it's just a very poorly implemented AI bot, so it probably just scans tags etc and a human only gets to assess it if the AI gets confused ! It also doesn't consider context nor is it able to see larger patterns of behaviour, hence it's so difficult to successfully report abusive players and trolls as individual comments etc appear harmless, only repetitive behaviour presents a problem which Sony moderation seems incapable of detecting!
Anyway, Tldr; I sympathise with your ordeal and think your suggestions are perfectly reasonable and logical.
Just had that happen to me. taken down for the same thing too. My creation had no violence of any kind, yet it still got taken down.

@TAPgiles: Thanks for your answer! I tried to open it. The scene and the dream are flagged as moderated. I can't open previous versions and unfortunately there is no remix button. :(
I'm not sure what you mean with appeals process. But anything different from the current moderation process would be an improvement I guess, since currently you can't do anything when some creation of you gets reported. And even if you try your best to not violate the policies you import something of another dreamer and get the same punishment.
And I really don't wanna get banned for something someone else did and which is really discussable like this example.
I believe even if something is moderated you can remix it and change it and release it again.
The moderation is done by Sony I believe? Mm want to put an appeals process in place.
